At Unique Dental of Putnam, we strive to make each of your visits as smooth as possible. Your First Visit
Each of our new patient appointments is an hour long, as we want to allot the proper amount of time to get to know you and make sure we conduct a thorough initial examination. As soon as you enter our practice doors, you’ll be greeted by one of our friendly front desk staff who will gather your new patient forms, confirm your dental insurance provider, and guide you back to one of our treatment areas.
First, our dental assistant will gather all necessary diagnostic information, including X-rays, and intraoral scans and photos of your mouth. Before your cleaning, we like to interview our new patients to better understand if they’ve had any bad past experiences so we don’t do anything that will make you feel uncomfortable or fearful.
Once your cleaning is complete, you’ll receive an in-depth evaluation from either Dr. Moaz or Dr. Zakhem, as well as a walkthrough of any treatment recommendations. Before you leave, our team will be sure to schedule your next routine appointment or any necessary follow-up visits.

Dental Insurance
We are proud to be in-network with multiple major dental insurance companies and plans. If you find that we aren’t in-network and have a PPO dental insurance plan, you can still come to our office! Our team will file claims on your behalf to help you maximize your benefits and minimize any out-of-pocket costs. If you have any questions about your coverage, feel free to call our front desk.
In-House Dental Membership Plan
If you’re uninsured, you can take advantage of our in-house dental membership plan by paying a flat annual fee. The plan includes all of your basic preventive services for the year as well as a 10 percent discount on all other treatments you require. Plus, you don’t have to worry about maximums, deductibles, or waiting periods!

We understand that taking care of your smile isn’t always easy financially, which is why we offer flexible financing through CareCredit. Upon approval, CareCredit will split up the cost of your treatment into small, more manageable monthly amounts at little-to-no interest.
